Stage 3 – Information Transfer upon Your Authorization

Only after you have reviewed the recommended options and selected a specific doctor, hospital, or medical institution will we seek your separate authorization to transfer relevant medical information to that selected provider. Such transfer will occur only after you have given your authorization.
